Job summary

Compassus in Memphis is seeking a Hospice Admissions Coordinator to process patient referrals, coordinate admissions, and enter patient admission data after all hospice requirements are met. The role operates Monday through Friday, 8am–5pm, based at the Memphis office.

You will work with physicians, nurses, and the care team to manage admissions, secure orders, coordinate pre-certifications, and communicate scheduling and information to patients and families, ensuring a smooth transition into

Qualifications

• High school diploma or GED required; Bachelor's degree preferred.
• 2-3 years of related experience strongly preferred.
• Experience communicating with physicians, nurses and other healthcare personnel is highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsibilities

• Process patient referrals from physicians, families and community.
• Coordinate admission of patients and enter admission data after all requirements are met.
• Notify interdisciplinary team (IDT) and ancillary providers of admission details.
• Ensure proper admission documentation is signed, dated, and filed.
• Market referral sources to promote hospice services.
• Provide back-up support for Hospice Care Consultant.
• Perform other duties as assigned.
