Position Summary

The Hospice Admissions Coordinator is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and fostering positive working relationships within the company, among all departments, and with external stakeholders. The Hospice Admissions Coordinator processes patient referrals, coordinates patient admissions, and enters patient admission data after all hospice requirements are met.

An LPN is preferred for this position. Flexible days with working hours from 11 am to 7 pm, covering weekends.

Position Specific Responsibilities
  • Attend and participate in staff meetings, educational programs, and community events as requested.
  • Communicate identified needs and potential solutions to the supervisor.
  • Receive and process referrals from physicians, patients, families, friends, and the community.
  • Contact physicians to secure an order for admission. If the Admissions Coordinator is not a Registered Nurse, ensure a nurse is present to take a verbal order or request a faxed script.
  • Contact insurance companies for pre-certification authorization.
  • Schedule appointments with patients and families for informed consents and RN assessment visits.
  • Explain hospice services and benefits to patients, family members, and significant others as needed.
  • Inform appropriate team members of appointment times.
  • Complete the admission process according to agency/corporate policy.
  • Notify the interdisciplinary team (IDT) and ancillary providers of admissions.
  • Enter admission data into a third-party system.
  • Ensure patient charts are complete, signed, and dated; then hand over to the Team Coordinator.
  • Market referral sources.
  • Contact long-term care staff, hospital staff, and physicians to promote hospice services.
  • Provide backup support for the Hospice Care Consultant.
  • Document marketing contacts as directed.
  • Perform other duties as assigned.
Education and Experience
  • High school diploma or GED required.
  • Bachelor's degree preferred.
  • Two to three years of related experience and/or training strongly preferred; an equivalent combination of education and experience will be considered.
  • Experience communicating with healthcare professionals highly preferred.
  • Experience in insurance, Medicare/Medicaid billing, and reimbursement preferred.
Skills
  • Mathematical Skills: Ability to perform basic math operations and compute rates, ratios, and percentages.
  • Language Skills: Ability to read, analyze, and interpret various documents; write reports and correspondence; and present information effectively. Strong communication skills required.
  • Other Skills and Abilities: Proficiency in English; ability to articulate and embrace hospice philosophy.
Physical Demands and Work Environment
