Job summary

VITAS Healthcare is seeking a proactive Care Connection Center Coordinator to manage referrals, schedule admissions, and support patient access. You will verify benefits, coordinate with admissions and marketing, and maintain data accuracy to maximize timely admissions.

The role requires strong communication, organizational skills, and the ability to work weekends as needed. Bilingual English/Spanish is a plus, with high school completion and familiarity with call-center environments.

Qualifications

• Two or more years related experience in medical terminology and/or in a call center environment.
• Ability to attain goals in a fast-paced, dynamic environment.
• Ability to exercise judgment and discretion in partnering with internal and external callers.
• Superior communication to provide concierge level service, in writing and verbally.
• Proficient in computers, internet, and navigating automated systems; 45 wpm typing; collaboration with leaders.

Responsibilities

• Process new referrals by telephone, email, fax, and website inquiries.
• Follow up on multiple lists of pending patient charts daily.
• Enter information to support data integrity and maximize reimbursement.
• Verify Medicare and Medicaid hospice benefits.
• Notify Managed Care Department of commercial insurances and plans.
• Assign appointments to Admissions Representatives and provide reports after hours.
• Complete admission consent paperwork as needed.
• Update referral sources on status of referred patients.
• Establish and maintain relationships with internal and external customers.
• Provide training and mentoring to new team members.
