Three Oaks Hospice, Inc. is seeking an experienced Registered Nurse to perform admissions and provide in-home nursing care for patients across ages.

You will coordinate care with the interdisciplinary team and referring agencies, ensuring comprehensive, compassionate service in the patient's residence. The role requires a current RN license, state licensure, and at least two years of clinical nursing experience with hospice exposure.

Qualifikationen

• RN must be graduate of an accredited nursing program.
• Current RN license; valid driver's license.
• Two years nursing experience in a clinical care setting; hospice experience preferred.

Aufgaben

• Assess patients using Nursing Assessment and LCD criteria to identify needs.
• Develop and document the care plan based on assessment findings.
• Provide nursing care with infection control measures.
• Coordinate with Interdisciplinary Group to meet patient and family needs.
• Communicate with attending hospice physicians and other involved physicians.
• Submit required paperwork in a timely manner.
• Demonstrate commitment to professional growth and competency.
• Meet productivity standards and provide on-call coverage as assigned.
• Communicate effectively with patients, families, team members and other health professionals.
