Job summary

Three Oaks Hospice in Richardson, TX is seeking a compassionate Registered Nurse to perform admissions and administer nursing care for patients in their homes, coordinating care with the interdisciplinary team and families.

You will communicate with attending physicians, complete required paperwork timely, meet productivity standards, and provide on-call coverage as assigned while upholding professional standards and patient-focused care.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse and valid driver's license.
• Two years RN experience in a clinical care setting and 1 year full-time hospice.
• Knowledge of state and federal regulations; good interpersonal skills.
• Reliable transportation. Driver’s license and auto liability insurance.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify the physical, psycho social, and environmental needs.
• Develop and document the plan of care in correlation with Assessment.
• Work with the Interdisciplinary Group to ensure patient/family needs are met.
• Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
• Timely submission of all required paperwork.
• Demonstrate commitment, professional growth and competency.
• Meet productivity standards.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Provide effective communication to patient/family, team members, and other health care professionals
