Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for Home Health in Mechanicsburg, PA. This staff position offers 40 hours per week, 8-hour shifts, travel to patient homes, and ongoing professional autonomy within an interdisciplinary team.

Admission RN duties include comprehensive hospice assessments, care planning, patient and family education, and precise EMR documentation. A $10,000 first-year bonus is available to reward early achievements.

Qualifications

• Active RN license in PA.
• Two years RN experience; hospice experience preferred.
• Valid driver’s license and ability to drive ~20% of the day.
• Ability to travel to patient homes within assigned territory.
• Drug screen required as part of employment background check.

Responsibilities

• Deliver compassionate hospice care in home settings with the interdisciplinary team.
• Conduct admission assessments and develop plans of care.
• Educate families about the hospice admission process and expected care.
• Ensure accurate EMR documentation and timely admissions.
• Coordinate with other disciplines as needed and initiate evaluations.
