Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for home-based patient care in Penns Creek, PA. The role involves end-of-life care, interdisciplinary collaboration, and travel to patient homes within the territory.

Qualified candidates have an active RN license in the state, at least two years of RN experience (hospice preferred), and a valid driver’s license with ability to travel ~20% of the day. Comprehensive benefits and growth opportunities are offered.

Responsibilities

Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team.
Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
Educate and support patients and families through the end-of-life journey, ensuring they feel informed and cared for.
Think critically, ensure compliant EMR documentation.
Work independently, but with team support.
Travel to patient homes within assigned territory.
