Qualifications

• Active RN license in the state you work.
• Two years RN experience with hospice experience preferred.
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care in home settings with an interdisciplinary team.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed and cared for.
• Think critically and ensure thorough EMR documentation.
