• Driver's license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Deliver compassionate hospice care to patients in home settings.
• Coordinate with an interdisciplinary team to manage symptoms and plan end-of-life care.
• Respond to after-hours needs while traveling to patient homes within territory.
• Document care and maintain compliant EMR records.
