Graham Healthcare Group in Exton, PA is seeking a Registered Nurse (RN) for Hospice to provide end-of-life care in patients' homes. This ongoing staff position offers 40 hours per week, with 8-hour shifts and travel to patient homes within the territory.

The role emphasizes compassionate, patient-centered care with an interdisciplinary team, thorough assessments, and clear EMR documentation. A $10,000 first-year success bonus is available to reward milestone achievements.

Qualifications

Active RN license in the state you work
Two years of RN experience recommended, hospice experience preferred
Driver's license and ability to travel ~20% of workday

Responsibilities

Deliver compassionate, patient-centered hospice care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
Work independently, but never alone
Travel to patient homes within assigned territory
