Job summary

Arkansas Hospice, Inc. in Batesville, AR is seeking an Access Registered Nurse (RN) to coordinate inquiries, referrals, and admissions to hospice care.

As part of the access team, you will interact with discharge planners, physicians, and staff nurses to evaluate patient eligibility, coordinate admission, and may provide care for GIP patients at contracted hospitals. The role requires strong communication and confidentiality, with a focus on compassionate, patient-centered care.

Qualifications

• Education: Graduation from an accredited nursing school.
• Experience: A minimum of one year experience as an RN in a hospital, home health, hospice, or related program.
• Licensure: Licensed as an RN in Arkansas.

Responsibilities

• Greets customers on the telephone, records patient information, and enters referral calls into the electronic record.
• Serves as a resource for information and support to referred patients, families and referral sources.
• Evaluates information received from referral source and assesses patient for eligibility for hospice services and level of care.
• Ensures timely transfer of all admission information to appropriate team members.
• Completes admission process and required documentation in the electronic health record.
• Assesses family and caregiver(s) for response to hospice care and communicates expectations to the patient and family.
