Job summary

Bristol Hospice Oklahoma LLC is seeking an Admissions RN to build relationships with referral sources and improve access to hospice care in the Fort Gibson area. You will lead admissions, assess patient needs, and coordinate with EDs and community partners to ensure timely, compassionate care.

The role requires a current RN license, a year of recent nursing experience, and the ability to travel up to 20% with flexible hours to meet patient needs.

Qualifications

• Must have a current RN license in the state.
• Minimum of 1 year of nursing experience in the last 3 years, or a BSN from an NLN-accredited program.
• Willingness to travel ~20% of the time and adapt to flexible hours.

Responsibilities

• Establish relationships with referral sources to promote hospice access.
• Educate the community about hospice care and Bristol Hospice services.
• Coordinate admissions, assessments and care plans with agencies and EDs.
• Review patient information to determine hospice needs and criteria.
• Serve as liaison between facility, physicians, hospitals and hospice teams.
• Participate in IDT meetings and contribute to patient/family plan of care.
• Provide professional nursing care using the nursing process and health assessments.
• Support ongoing education programs and QA/QPI initiatives as needed.
