Job summary

Arkansas Hospice, Inc. is seeking a Registered Nurse Case Manager in Batesville, AR. This full-time professional will oversee clinical interventions and support for an assigned hospice caseload, ensuring compassionate, patient-centered care.

The role requires an RN in Arkansas with a valid driver's license and the ability to coordinate with an interdisciplinary team, maintain Plan of Care updates, and participate in on-call rotations to serve patients and families 24/7.

Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Minimum of one year nursing experience in a hospital, home health, hospice, or related program.
• Licensed RN in Arkansas.
• Valid Arkansas driver’s license, personal automobile, and automobile insurance required.

Responsibilities

• Manages patient care functions after admission and assignment to an RN Case Manager.
• Assists in the admission process as requested.
• Coordinates with interdisciplinary team to manage the plan of care continually.
• Updates the Plan of Care and keeps team informed of patient status and needs.
• Facilitates communication with patients, families, and other providers.
• Rotates on-call duty to provide 24-hour service to patients and families.
