Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Minimum of one year nursing experience in hospital, home health, hospice, or related program.
• Licensed as a Registered Nurse (RN) in Arkansas.
• Valid Arkansas driver’s license, personal automobile, automobile insurance required.

Responsibilities

• Manages patient care functions after admission and assignment to a designated RN Case Manager.
• Assists in the admission process as requested.
• Interprets Arkansas Hospice, Inc. policies for nursing staff and others to ensure understanding.
• Coordinates with interdisciplinary team to manage the plan of care.
• Updates the Plan of Care on an ongoing basis and communicates progress to team.
• Maintains effective written and verbal communication with patients, families, and other providers.
• Monitors patient status and coordinates resources for care.
