Southwest Behavioral & Health Services seeks a Housing Specialist for our HOPWA program in Phoenix, AZ. The role supports eligibility screening, housing placement, and ongoing coverage for residents in need.

Expect a Monday–Friday schedule from 8:00 AM to 5:00 PM, with collaboration across interdisciplinary teams to develop individualized housing plans and ensure housing stability and safety.

Qualifications

• Must be at least 21 years old.
• High school diploma/GED with 2 years behavioral health/integrated healthcare experience, OR
• Associate's degree with 1 year relevant experience.
• Related Associate’s degree with 6 months relevant experience.
• Valid Arizona driver's license and ability to pass a motor vehicle screen.
• Fingerprint clearance issued by the Arizona DPS or eligible to receive on hire.
• Must pass a 39 month MVR clearance.

Responsabilités

• Performs intakes, screening and assessments on residents.
• Meet residents in transitional housing 2x per month or more to assess needs.
• Oversee housing unit safety; inspect detectors; review emergency plans.
• Identify unmet needs; provide engagement, referrals and follow-up.
• Develop individualized housing plans with residents.
• Intervene in crisis and coordinate with supports as needed.
• Consult landlords and management about responsibilities to units.
• Ensure HQS housing standards and rent procedures are followed.
