Job summary

Primary-Health-Care, Inc. in Des Moines, IA seeks a Homeless Support Programs Manager to lead grant-funded efforts providing case management and housing services for individuals experiencing homelessness.

You will oversee daily operations and a team of 10-15 staff, ensuring high-quality services through collaboration and compliance. This role typically works at PHC University Campus in Des Moines but travels to off-site locations as needed.

Qualifications

• Bachelor's degree in health care, human services, social work, psychology, counseling, mental health, or a related field.
• Two years of experience with local housing or community resources.
• Must obtain Mandatory Reporter Training for Child/Dependent Adult Abuse within 90 days of employment.
• State of Iowa Chauffeur’s license and a valid driver’s license.
• Strong coaching, organizational, and communication skills; ability to work independently.

Responsibilities

• Oversee day-to-day operations of the Homeless Support Services Department.
• Lead and develop program staff, set expectations, provide training, and conduct evaluations.
• Monitor grant revenues/expenditures, manage the program budget, and attend budget meetings.
• Build relationships with community partners and funding agencies; conduct outreach and advocacy.
• Participate in grant writing and program reporting; ensure compliance with grant and PHC requirements.
• Drive performance improvements and support clients across the care continuum.
• Assist clients in accessing PHC services and address barriers to care.
