Qualifikationen

• Bachelor's degree in social services, human services or related field; or equivalent experience. Certification preferred.
• Minimum of one (1) year practical experience in social services or with the homeless population. Two (2) years’ experience may substitute for education.
• Valid Florida driver's license, adequate car insurance and clean driving record.

Aufgaben

• Assess residents and families for appropriate services and formalize individual service and goal plans.
• Participate in case conferences regarding resident status, needs, outplacement, and discharge plan.
• Provide services to restore residents to an optimal social environment, utilizing community agencies and resources.
• Provide outplacement and follow-up services with residents.
• Prepare and maintain a case record on each resident in HMIS.
• Prepare statistical reports and a variety of reports for distribution.
• Manage customer satisfaction surveys and analyze productivity and performance reports.
• Assist with transportation needs for residents; inspect vehicles and report maintenance needs.
