Job summary

Primary Health Care (PHC) is seeking a Program Support Specialist for Homeless Support Services in Des Moines, IA. You will provide administrative support, assist clients with basic needs, and enter data into HMIS to help programs meet grant requirements.

You will collaborate with PHC staff and external agencies, delivering excellent customer service while maintaining accuracy and empathy in all interactions. A high school diploma and healthcare/customer service experience are required.

Qualifications

• High school diploma or equivalent.
• 1 year of experience in healthcare or human services; or 2 years in customer service.
• Proficiency with basic computer applications (email, word processing, spreadsheets).
• Fluency in English; Spanish proficiency preferred.

Responsibilities

• Welcomes clients and visitors; provides prompt, friendly service and guides to resources.
• Completes accurate client registrations for services.
• Answers calls and handles messages per protocol.
• Assists clients with basic needs (shower, clothing, supplies).
• Advocates for clients; accesses community resources and external referrals.
• Enters case notes and program data into HMIS per grant guidelines.
• Performs other duties as assigned.
