Job description

  • Develop and implement homecare services for PACE program participants
  • Assess homecare needs of a frail elderly population and develop plans of care
  • Collaborate with other disciplines and the Interdisciplinary Team to provide quality services
  • Monitor patient, family, and caregiver needs and problems
  • Coordinate and authorize durable medical equipment, consumable supplies, environmental adaptations, and other home services
  • Coordinate outside agency staffing schedules and agency education for PACE requirements
  • Enter and maintain patient care, demographic, timesheet, careplan, and report data
  • Generate and distribute accurate reports, lists, and updated information
  • Participate in Interdisciplinary Team meetings
  • Review PACE Home Services invoices and monitor requisitions
  • Conduct annual Home Care Service Agency audits
  • Process homecare service requests and coordinate delivery and pickup of DME and supplies
  • Provide internal and external customer service and assist staff with documentation and information
  • Conduct unscheduled home visits as requested by the Interdisciplinary Team
  • Assist with special projects and other assigned duties
  • Represent the company professionally and complete responsibilities in accordance with organizational policies and safety requirements
Requirements
  • High school diploma or GED required
  • One or more years' experience with a frail or elderly population
  • Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs
  • Valid Florida driver's license
  • Current auto insurance and reliable transportation
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Ability to perform mathematical calculations and interpret graphs
  • Ability to solve problems with limited standardization
  • Ability to interpret written, oral, diagram, or schedule instructions
  • Ability to communicate, concentrate, document, read, write reports, and speak effectively
  • Proficient computer skills
  • Ability to occasionally lift and/or move up to 50 pounds
  • Consent to drug and/or alcohol testing after a conditional offer
  • Ongoing compliance with the Drug-Free Workplace Policy
  • Eligibility through the Florida Care Provider Background Screening Clearinghouse
