Responsibilities

  • Maintain van schedule and driver availability.
  • Advise IDT and manager of participant-specific transportation issues.
  • Develop, update and maintain transportation routes.
  • Dispatch drivers; coordinate internal and vendor transportation for all participant appointments.
  • Adjust schedules to meet unanticipated changes and determine the safest and most efficient pick-up and delivery routes.
  • Participate as a member of the interdisciplinary team.
  • Take part in participant care planning and contribute to notations in the medical record.
  • Report important information to proper Hope PACE staff daily regarding changes, progress, or unusual events concerning participants.

Requirements

  • High school diploma or general education degree (GED).
  • At least one year experience in transporting handicapped individuals and working with the frail elderly.
  • Valid Florida Drivers’ License with good record
  • Automobile Insurance
  • Clear level 2 background check
  • Clear communication; via phone, email, and in person.
  • Proven attention to detail.
  • Demonstrate time management and organizational skills.
  • Must be flexible to work in other locations if needed.
  • Consent to drug and/or alcohol testing after a conditional offer of employment.
  • Ongoing compliance with the Drug-Free Workplace Policy.
  • Eligibility through the Florida Care Provider Background Screening Clearinghouse, with ongoing eligibility.
