Chapters Health in Florida seeks a Home Care Coordinator to develop and implement homecare services for program participants. You will assess homecare needs of a frail elderly population, identify care plans, and collaborate with other disciplines to ensure quality services.
You will coordinate scheduling of outside agency staffing, manage data entry, timesheets, and care plans, and communicate with the IDT to support patient/family needs.

Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
High school diploma or general education degree (GED) is required. Must have one or more year's experience with a frail or elderly population. Experience working with outside agencies and customer service experience is preferred.
Must possess a valid Florida driver's license, current auto insurance, and reliable transportation.
Active BLS for healthcare professionals from the American Heart Association or Red Cross.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Ability to solve problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to communicate, concentrate and document effectively and accurately.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
