What You’ll Be Responsible For
The Home Care Manager oversees the day-to-day operations and operational effectiveness of the Home Care department to ensure high quality, safe, effective care is provided in conformance with policies, procedures and standards and all governing state and federal regulations. Participates in the Interdisciplinary Team (IDT) Meeting as required, acting as the Home Care representative.

As a Program of All-Inclusive Care for the Elderly (PACE) provider and the first of its kind in Western Colorado, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs. Qualifications
:
:
Bachelors or better.
1 year: Working with the frail or elderly population.
Home Health Experience.
2 years: Management experience in a hospital, nursing home or community-based setting.
Driver's License
Physical Therapist
Occupational Therapist
Cert. Nursing Assistant
Licensed Practical Nurse
Registered Nurse
