Job summary

Ennoble Care LLC in Philadelphia, PA seeks an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative home-based care to patients in the region on a full-time basis.

You will perform physical exams, develop care plans, order diagnostics, prescribe medications, and coordinate with physicians and families to ensure compassionate, patient-centered care in the home setting.

Qualifications

• Board-certified NP or PA in PA is required.
• 1–2 years of primary care experience; hospice experience preferred.
• Strong clinical skills and ability to work independently.
• Positive demeanor and good people skills.

Responsibilities

• Provide high quality primary and palliative care to homebound patients in the Philadelphia area.
• Perform physical examinations and assess medical histories to create care plans.
• Order diagnostic testing as needed and prescribe medications and interventions.
• Communicate with office staff and clinical operations across internal platforms.
• Develop impressions, diagnoses, and treatment plans; coordinate with patients, families and caregivers.
