Job summary

Ennoble Care seeks an experienced Nurse Practitioner or Physician Assistant to provide primary and palliative home-based care in Cumberland County, NJ. You will work under physician supervision to care for a diverse set of patients with chronic diseases.

You will assess, diagnose, and manage patient plans, collaborate with the in-house clinical team, and participate in meetings and care coordination. A flexible, autonomous role with growth opportunities is offered.

Qualifications

• Board Certified, with a valid NP or PA certification in New Jersey.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.
• Strong clinical skills with the ability to assess, diagnose, and treat independently.

Responsibilities

• Provide primary and palliative care to homebound patients in Cumberland County, NJ.
• Perform physical examinations and develop care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Attend quarterly medical staff meetings.
• Order diagnostic testing and prescribe medications as needed.
