Job summary

Ennoble Care is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients around Philadelphia, PA, on a full-time basis. You will apply medical techniques under physician supervision to care for patients with chronic diseases.

You will assess health through exams and histories, create care plans, and collaborate with the clinical team to ensure high-quality, patient-centered care across platforms.

Qualifications

• Board Certified with a valid NP or PA license in PA.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality primary and palliative care to homebound patients in the Philadelphia, PA region.
• Perform physical examinations and develop care plans.
• Communicate with office staff and clinical operations across platforms.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients and families during visits; build trust.
• Order diagnostic testing as needed and prescribe medications.
