Job summary

Bloom Healthcare in St. Louis, MO is seeking a Primary Care Nurse Practitioner or Physician Assistant to deliver patient-centered care at home and in communities. This role emphasizes independence with a supported interdisciplinary team and advanced data systems to improve outcomes.

You'll care for homebound seniors with a focus on chronic and acute conditions, social determinants of health, and meaningful, relationship-driven care that keeps patients safe at home.

Qualifications

• NP with master's degree and Missouri licensure.

Responsibilities

• Provide longitudinal, holistic medical care in patient homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health and access barriers.
• Conduct wound care, post-hospital care, and condition-specific assessments.
• Coordinate post-discharge visits within 72 hours.
• Collaborate with care teams to reduce readmissions and stabilize high-risk patients.
