Job summary

Bloom Healthcare in St. Louis, MO is seeking a Primary Care NP to deliver longitudinal, home- and community-based primary care to medically complex seniors. You’ll practice independently with strong interdisciplinary support and data-driven care delivery.

This role offers a manageable patient load, Monday–Friday schedule, and a comprehensive benefits package, including immediate health coverage and PTO, with an emphasis on autonomy, meaningful outcomes, and patient-centered care.

Qualifications

• Must hold NP with Missouri licensure.
• Experience serving seniors and medically complex patients preferred.
• Comfortable working in home-based settings.
• Ability to coordinate care with interdisciplinary teams.

Responsibilities

• Serve as the primary care provider for a defined patient panel in homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health (housing, nutrition, caregiver support).
• Conduct wound care, post-hospital transitions, and condition-specific assessments.
• Perform comprehensive evaluations including med reconciliation and fall risk assessments.
• Use Bloom's data ecosystem to track hospitalizations and coordinate post-discharge visits.
• Collaborate with internal and external teams to reduce readmissions and stabilize high-risk patients.
