Qualifications

• Must hold an NP license in Missouri or be licensed as a Physician Assistant in Missouri.
• Experience delivering value‑driven, relationship‑based primary care.
• Comfort working autonomously in home and community settings.

Responsibilities

• Serve as the primary care provider for homebound patients, delivering longitudinal, holistic care.
• Diagnose, manage, and treat chronic and acute conditions with support from a multidisciplinary team.
• Address social determinants of health and coordinate post‑discharge care.
• Conduct in‑home visits such as wound care, chronic disease management, and preventive exams.
• Perform comprehensive evaluations including med reconciliation, fall risk, and care planning.
