Job summary

Signify Health is seeking a full-time physician to conduct in-home health evaluations across patients in the Cedar Rapids area. The clinician will review medical history, perform physicals, and coordinate with health plans for diagnostics, all within patients' homes.

The role requires MD/DO credentials with an active license, and involves travel to local residences. You will use a tablet-based workflow to document findings and ensure high-quality care delivery.

Qualifications

• Active, unrestricted license(s) in the coverage area(s).
• Board certified in family medicine or internal medicine preferred.

Responsibilities

• Provide in-home health evaluations to patients.
• Engage with patients to review medication and medical history.
• Perform physical evaluations and coordinate with health plans for additional testing.
• Travel locally to patient homes as needed.
• Utilize digital tools (iPad) for data collection and workflow.
