Job summary

Signify Health, a CVS Health company, seeks a Per Diem Physician to deliver In-Home Health Evaluations in Cedar Rapids, IA. Clinicians visit members at home, perform medical history, physicals, and may arrange tests as needed. Travel locally within licensed states is required and work includes virtual visits when appropriate.

Hourly base pay is $105, with additional incentives per visit. This part-time role offers malpractice insurance and a benefits package for eligible hours.

Qualifications

• Active, unrestricted medical license in coverage area(s)
• Experience conducting in-home evaluations or house calls
• Ability to travel locally within licensed states and provide virtual visits as needed

Responsibilities

• Perform in-home health evaluations including medical history, physical examination and, if ordered, required diagnostic tests
• Bridge gaps in care by coordinating with patients, providers and payers
• Build trusted relationships with members and their families to support health outcomes
• Travel locally within licensed states; conduct virtual visits when appropriate
