Abode Care Partners is seeking a Nurse Practitioner to provide primary care to seniors and patients with complex needs in their homes. This role emphasizes autonomous practice, collaboration with interdisciplinary teams, and chronic disease management in various community settings.

You will conduct clinical assessments, develop personalized care plans, and participate in a quarterly incentive program that enhances total compensation through performance-based rewards.

Qualifications

• Active state NP license.
• Current NP national board certification.
• Active state DEA licensure.
• Master's degree in Nursing (MSN).
• Five or more years of nursing experience; at least two years NP experience preferred.
• Strong interpersonal communication skills – verbal and written.
• Excellent time management skills with accountability for daily workflow.
• Proficiency in EMR systems and Microsoft Office applications.
• Licensed driver with valid automobile insurance.
• Ability to engage in moderate physical activity and lift up to 25 lbs.

Responsabilités

• Plans, directs, and evaluates patient medical needs.
• Examines and treats chronic and episodic illnesses and injuries.
• Orders and interprets laboratory and diagnostic testing.
• Collaborates with caregivers and other health professionals on care plans.
• Provides referrals to other practitioners when necessary.
• Maintains up-to-date patient medical records.
