Job summary

Optum Home & Community Care in the United States seeks a HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and coordination with primary care providers to improve patient outcomes.

Flexible scheduling from 8am-8pm, 24-40 hours per week, with no on-call, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state without disciplinary actions.
• Driver’s license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments, including history, medications, vitals, and exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
