Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. The role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.

You will work in a flexible, field-based environment with no on-call, weekends, or holidays, and may access language-augmented compensation for non-English speakers.

Qualifications

• Active ANCC or AANP certification as Family/Adult/Geriatric/Adult-Gerontology Primary Care.
• NCCPA certification and PA state licensure.
• Active, unrestricted licensure in the practicing state and ability to obtain licensure by start date.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and treatment planning; communicate findings to PCPs.
• Identify urgent/emergent situations and intervene appropriately.
