UnitedHealth Group is seeking an in-home Advanced Practice Clinician (NP or PA) to conduct annual health assessments for Medicare Advantage members in home settings. This field-based, non-prescribing role focuses on education, gap closure, and collaboration with PCPs.

Flexible scheduling, no on-call, weekends, or holidays required, with 8am-8pm coverage options and 24-40 hours per week.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Ger iatric, or Adult-Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted licensure where practicing, no disciplinary actions.
• Active and unrestricted driver’s license with reliable transportation for home visits.

Responsabilités

• Conduct comprehensive in-home assessments of Medicare Advantage members.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address care gaps.
• Recognize urgent or emergent situations and respond appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
