Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members and other plan members. This field-based role emphasizes education, gap closure and collaboration with PCPs, with no prescribing required.

The position supports flexible scheduling, requires licensure in the practicing state, and offers comprehensive benefits and career development within Optum HouseCalls.

Qualifications

• NPs: Active ANCC or AANP Family/Adult/Geriatric certification.
• PAs: NCCPA certification and state licensure.
• Active, unrestricted state license; ability to obtain by start date.
• Valid driver's license and reliable transportation for home visits.
• Ability to work in varied home environments; sit/stand/kneel as needed.

Responsibilities

• Conduct in-home assessments including history, vitals, medication reconciliation and exam.
• Identify diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps.
• Recognize urgent situations and intervene as needed.
• Educate on disease processes, medications and adherence.
• Address social determinants of health and provide referrals.
