Job summary

Optum Home & Community Care, part of the Optum family, is hiring an Advanced Practice Clinician (NP or PA) to perform in-home Medicare assessments. This field-based role emphasizes education, gap closure, and coordination with primary care providers to improve patient outcomes.

Flexible scheduling is offered across 8am–8pm over a 7-day week, with 20–40 hours. A $10,000 sign-on bonus and comprehensive benefits are provided.

Qualifications

• Active certification as NP or PA in the applicable scope.
• Unrestricted state license or ability to obtain by start date.
• Driver's license and access to reliable transportation for home visits.

Responsibilities

• Conduct in-home assessments including medical history, vitals, and exams.
• Document diagnoses and support care planning with PCPs.
• Educate patients on diseases, medications, and adherence.
• Identify urgent situations and respond appropriately.
• Address social determinants of health and refer to needed services.
