Job summary

PruittHealth is seeking an LPN Assessment Nurse to conduct initial assessments and reassessments for elderly and disabled waiver clients in home settings. You will review medications, document findings, and coordinate with the Medical Director and Home First team to support client goals.

The role emphasizes accuracy, timely documentation, and adherence to safety and regulatory guidelines, with ongoing participation in team meetings and in-services.

Qualifications

• Must be computer literate and able to learn new programs.
• Two (2) years practical nursing experience required.
• Experience working with elders and disabled populations.
• Must be able to communicate with translators for non-English-speaking clients/members.
• Keeps supervisor informed of daily activities and issues.

Responsibilities

• Completes initial and annual assessments in client/home and determines ADL/IADL abilities.
• Completes modified assessments for transfers, hospital/nursing home discharges, and added services.
• Ensures services to keep members in the community and meets fiduciary obligations.
• Communicates monthly with administrator for reassessment schedule.
• Reviews medications for appropriateness and notes changes.
