Job summary

PruittHealth Corporation seeks an LPN Assessment Nurse to perform initial and reassessment visits in clients' homes under the SOURCE program. You will determine the member’s ability to complete ADLs/IADLs and coordinate services to maintain community living arrangements.

The role requires documenting in the Harmony system, coordinating with Medical Director and Home First team, and complying with regulatory guidelines.

Qualifications

• Nursing degree from an accredited college or university or LPN program.
• Minimum of two years practical nursing experience; elder/disabled population.
• Current and unrestricted LPN licensure in state of practice.
• Three years practical nursing experience preferred; waiver program experience advantageous.

Responsibilities

• Performs initial and annual assessments in clients' homes and assesses ability to perform ADLs/IADLs.
• Completes modified assessments for transfers, hospital and nursing home discharges.
• Ensures services are in place to keep members in the community and meets needs per guidelines.
• Documents assessments in Harmony system and requests records as needed.
• Participates in weekly team meetings with Medical Director to review new admits, reassessments, and discharges.
