Job summary

PruittHealth Corporation seeks an LPN Assessment Nurse SOURCE to conduct initial and reassessment visits for elderly and disabled waiver program clients in their homes. You will determine the client’s ability to perform ADLs/IADLs and coordinate care with the team.

You will document findings in the Harmony system, review medications, and participate in weekly physician-led case reviews. Prior nursing experience with aging populations is preferred.

Qualifications

• Requires Nursing degree or LPN program completion.
• Two years of practical nursing experience, preferably with elderly/disabled.
• Current, unrestricted LPN licensure in state of practice.

Responsibilities

• Conducts initial and annual in-home assessments to determine client abilities for ADLs/IADLs.
• Performs modified assessments for transfers, hospital discharges, or increased services.
• Documents assessments in Harmony and requests records as needed.
• Participates in weekly team meetings with Medical Director to review admissions and discharges.
• Reviews medications for appropriateness and notes changes.
