Job summary

Pruitthealth Corporation is seeking an LPN Assessment Nurse SOURCE to perform initial and annual assessments in clients' homes under the elderly and disabled waiver program. The role involves evaluating IADLs/ADLs, coordinating services, and updating patient records using the Harmony system.

Qualified candidates will have an LPN license, at least two years of nursing experience with elders, and the ability to communicate with diverse populations.

Qualifications

• Computer literate and able to learn new programs.
• Communicate with translators for non-English-speaking clients.
• Maintain confidentiality of client information.
• Demonstrate reliable attendance and teamwork.
• Follow agency policies, laws, and accreditation requirements.

Responsibilities

• Completes initial assessments in clients' homes and determines ability to perform IADLs/ADLs.
• Performs modified assessments for transfers, hospital discharges, and added services.
• Ensures services are available to keep clients in the community.
• Communicates monthly with administrator for re-assessment schedules.
• Reviews medications for appropriateness and notes changes.
• Documents assessment entries in Harmony per guidelines and requests records as needed.
• Participates in weekly team meetings with Medical Director and relevant staff.
• Attends mandatory in-services and adheres to safety procedures.
• Reports hazards and follows fire, smoking, infection control rules.
• Keeps supervisor informed of daily activities and issues.
• Provides care in accordance with waiver program guidelines and policies.
