Job summary

PruittHealth Corporation is seeking an LPN Assessment Nurse to perform initial and reassessment visits in clients’ homes under the elderly and disabled waiver programs. You will review medications, document findings, and coordinate with the Medical Director and Home First team.

The role requires strong communication skills, accurate charting in Harmony, and commitment to community-based care with adherence to safety and regulatory guidelines.

Qualifications

• Nursing degree from an accredited college or university, or a graduate of an accredited/approved LPN program.
• Minimum of two (2) years practical nursing experience is required. Experience working with elders and disabled populations required.
• Current and unrestricted Licensed Practical Nurse (LPN) licensure in state of practice.
• Three (3) years practical nursing experience is preferred; waiver program experience is a plus.

Responsibilities

• Conduct initial and annual assessments in client/member’s home and determine ability to perform IADLs and ADLs.
• Review medications for appropriateness and document changes.
• Document assessment entries in Harmony system and request records as needed.
• Participate in weekly team meetings with Medical Director to discuss new admits, reassessments, discharges, and variances.
• Provide care in residence for waiver program guidelines and ensure services meet member needs.
• Adhere to safety, infection control, and compliance policies and regulations.
