Qualifications

• Must be a licensed practical nurse with valid state licensure.
• Minimum of two years practical nursing experience; elder/disabled population preferred.
• Ability to complete assessments and document in Harmony timely.
• Ability to work in clients’ homes and coordinate with care teams.
• Must be computer literate and learn new programs.

Responsibilities

• Conduct initial assessments in clients' homes and assess ADLs/IADLs.
• Perform reassessments and determine service needs.
• Ensure services meet community-based waiver guidelines.
• Communicate reassessment schedules monthly with administrator.
• Review medications for appropriateness and document changes.
• Document entries in Harmony per guidelines and request records as needed.
• Participate in weekly team meetings with Medical Director.
• Attend mandatory in-services and comply with policies.
• Adhere to safety, infection control, and hazard reporting.
• Maintain confidentiality and comply with codes of conduct.
