Job summary

Insight Global is seeking a dedicated Community Health Worker to join an interdisciplinary care team. You will work directly with members in their homes, healthcare facilities, and community settings to connect them with healthcare and social resources, and to support health goals.

Qualifications

• Valid driver's license
• Reliable vehicle
• Ability and willingness to routinely travel
• Comfortable conducting in-person visits in members' homes and community settings

Responsibilities

• Engage with members in homes, hospitals, provider offices, and community agencies
• Build professional, supportive relationships with members and families
• Accompany members to medical appointments and social service visits
• Coordinate logistics supporting care plans, including transportation and reminders
• Gather demographics, eligibility and enrollment history and current health status
• Maintain timely documentation within Care Management guidelines and collaborate with the ICT
