Job summary

Moms In Motion in Virginia seeks a Service Facilitator Case Manager to visit clients at home, help families navigate Medicaid Waiver programs, and write plans of care that truly make a difference.

You will work locally with a focus on compassionate, organized support, and you may work remotely from a laptop/tablet while traveling to client homes. A valid driver's license is required and no degree is necessary.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; we care more about heart and experience.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle (no client transport).
• Excellent written and verbal communication.
• Able to pass a background check and provide 2 professional references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes across VA.
• Guide families through Medicaid Waiver programs.
• Write plans of care and assessments that drive outcomes.
• Troubleshoot service authorizations, timesheets, and related issues.
• Build lasting relationships with families based on trust and respect.
