Job summary

Frontier Home Health is seeking a licensed Physical Therapist to provide skilled physical therapy evaluation and home-based treatment to patients in their residences under physician orders, in accordance with the individualized plan of care and all applicable regulations.

You will assess mobility, design and educate home exercise programs, coordinate care with physicians and caregivers, document progress in the EMR, and contribute to safety, independence and optimal outcomes in the home setting.

Qualifications

• Licensed Physical Therapist in good standing.
• Experience delivering therapy in home health or patient homes.
• Ability to perform comprehensive PT evaluations and develop plans of care.

Responsibilities

• Performs comprehensive physical therapy evaluations and ongoing assessments in patients' homes.
• Designs and implements individualized home exercise programs.
• Documents care, progress and OASIS where applicable in the EMR.
• Educates patients and caregivers on safety, transfers, and use of equipment.
• Coordinates with physicians, nurses and interdisciplinary team for optimal outcomes.
