Job summary

Career Links is seeking a compassionate Physical Therapist (PT) to provide home health services across assigned service areas. You will evaluate, develop, and implement individualized treatment plans for patients recovering from injury, surgery, or managing chronic conditions in their homes.

You will collaborate with physicians, nurses, and other professionals to deliver patient-centered care, document progress, and educate patients and caregivers on home exercise programs and fall/injury

Qualifications

Doctor of Physical Therapy (DPT) from a CAPTE-accredited program.
Current Iowa Physical Therapist License.
CPR/BLS certification or ability to obtain before start.
Valid driver's license and transportation to service areas.
Strong communication and documentation skills.

Responsibilities

Perform comprehensive physical therapy evaluations and assessments.
Develop and implement individualized treatment plans.
Provide interventions to improve mobility, strength, balance, and independence.
Educate patients and caregivers on home exercise programs and injury prevention.
Document evaluations, treatments, progress notes, and discharge summaries.
Collaborate with physicians, nurses, and other therapists.
Monitor progress and modify plans as needed.
Ensure compliance with Medicare/Medicaid and agency policies.
