Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients in a home health setting. The role emphasizes improving mobility, functional independence, and daily living skills using standard therapy practices and specialized programs.

Applicant must hold a current PT license in the employing state, have basic therapy knowledge, and possess a valid driver’s license with automobile liability coverage.

Qualifications

• Licensed as a Physical Therapist in the state of employment.
• At least 1 year of clinical experience preferred.
• Demonstrate knowledge and skill in current therapy practice.
• Must have a valid state driver’s license and automobile liability insurance.

Responsibilities

• Assess and treat patients to improve mobility and daily living skills.
• Utilize specialty programs such as Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, and Total Shoulder Replacement to support patient care.
