Job summary

Option Care Health is seeking a Clinical Transition Specialist in the United States. The role focuses on educating patients and facility staff about home health and alternate-site services, while coordinating with discharge planners to ensure smooth transitions.

You will build relationships with referral sources and work with Account Managers to grow referrals and achieve sales goals, with a salary range reflecting state pay transparency.

Qualifications

• Licensed RN or Pharmacist in the state of practice with at least 2 years in healthcare.
• Ability to establish relationships with diverse stakeholders and vendors.
• Familiarity with ACHC and URAC standards; strong customer service mindset.
• Proficient PC skills: email, internet, Word, Excel, PowerPoint.
• Valid driver's license and reliable transportation for facility visits.

Responsibilities

• Maintain and grow referrals to increase patient starts and sales.
• Assess patient suitability for alternative site care and coordinate with discharge planners.
• Educate patients, families, and facility staff on services and products.
• Provide in-person education to patients and caregivers for timely discharge.
• Communicate with medical teams, agency staff, and family throughout discharge.
• Collect and verify pre-admission information and signoffs.
• Collaborate with Account Manager and Regional Sales Director on growth plans.
• Coordinate with marketing to deliver caregiver education and promotions.
• Review patient records for history and payer information for registration.
