Option Care is seeking a Clinical Transition Specialist to educate patients about home infusion services. This role requires maintaining strong relationships with referral sources and assessing patients for alternative site care.

As a key point of contact, you will work closely with families, facility staff, and internal teams to ensure seamless transitions and effective treatment plans. Strong sales, organizational, and patient education skills are essential for success in this position.

Qualifications

• At least 2 years of experience in the healthcare industry.
• Licensed Registered Nurse (RN) or Pharmacist in the state of practice.
• Experience in growing service provider partnerships.

Responsibilities

• Proactively maintains and grows relationships with referral sources.
• Educates patients and families about services provided.
• Conducts assessments of patients for alternative site care.
• Coordinates care transition with referral sources and partners.
• Participates in data collection for therapy start and tracking.
