Job summary

Option Care Health is seeking a Clinical Transition Specialist (RN or Pharmacist) to educate patients, families and discharge staff about home infusion services and products. You will assess hospital referrals and coordinate preadmission information to ensure smooth placement in the home health setting.

You will build relationships with referral sources, work with Account Managers to grow growth, and help secure timely discharges by guiding patients through education and transition processes

Qualifications

• Licensed RN or licensed Pharmacist in the state of practice and at least 2 years of healthcare experience.
• Experience establishing relationships with individuals at all levels of the organization and with vendors.
• Experience providing customer service and ensuring quality of services.

Responsibilities

• Proactively maintains and grows relationships with referrals to increase sales and patient starts.
• Educates patients, families and facility staff about Option Care services.
• Assesses patients identified by hospitals to determine suitability for home/alternate site care.
• Collaborates with intake, case managers and discharge planners to ensure smooth transitions.
• Coordinates transfer orders via phone, fax or e-prescribing to support timely discharge.
